Job summary

Paycom - ATS is seeking a Community Health Worker to serve as a trusted liaison within a Federally Qualified Health Center and/or Behavioral Health setting.

You will provide outreach, health education, navigation, care coordination, and social support to improve outcomes while collaborating with medical and behavioral health teams.

This seasonal role requires a valid California driver's license and the ability to work with diverse populations in a trauma-informed, culturally responsive manner.

Qualifications

• High school diploma or equivalent required.
• Bilingual English/Spanish preferred.
• Must have a valid California driver's license, reliable transportation, and liability insurance if driving a personal vehicle on BHS business.

Responsibilities

• Provide outreach, engagement, education, advocacy, and support services to patients and community members.
• Assist patients with navigation of healthcare systems, referrals, social services, and community resources.
• Support care coordination activities including appointment scheduling, referrals, transportation coordination, and linkage to services.
• Document patient encounters, interventions, and referrals in the EHR and tracking systems.
• Collaborate with providers, behavioral health staff, care coordinators, and community partners to support patient care plans.
