Job summary

Telecare Corporation is seeking a Community Health Worker in California to help engage Managed Care Plan members and support care management through transportation to appointments and access to social services. This is a full-time role with daytime hours, Monday–Friday.

The position requires a high school diploma, two years of mental health or intensive case management experience, and willingness to use a personal or company vehicle for client transport.

Qualifications

• High School Diploma or GED required.
• Two (2) years' experience providing mental health or intensive case management services and coordination.
• Paraprofessional with or without lived mental health experience.
• Experience supporting individuals with physical health challenges to navigate the physical health care system.

Responsibilities

• Assist in outreach and engagement with eligible Managed Care Plan members.
• Support care management through transportation to appointments and social services.

Skills

High School Diploma or GED

Two years experience in mental health/

Paraprofessional with lived experience

Navigate health care system

CPR CPI First Aid certified
