Qualifications

• Associates degree in nursing or a related field.
• One year of nursing experience with the population served.
• Active LPN/LVN license in the state of residence and relevant certifications.

Responsibilities

• Provides prescribed medical treatment and personal care under RN/physician supervision.
• Completes health assessments and documents health diagnoses to support care plans.
• Monitors medications, communicates with pharmacy, and administers medications as required.
• Oversees staff administering medications and performs quality checks on records.
• Provides emergency coverage and coordinates with external agencies as needed.
• Maintains confidentiality and adheres to infection control and policy standards.

Education

Associates degree in nursing or related field

One year experience working with population served in a nursing capacity

Current LPN/LVN licensure for state of residence

CPR/FA and HIV/AIDS certification, BBP, and ACT training
