Job summary

Sevita is seeking a dedicated LPN/LVN to provide prescribed medical treatments and personal care to individuals in our community-based services. You will work under a RN or physician to monitor health, administer medications, and coordinate with care teams.

The role requires an Associate degree in nursing, current state licensure, and at least one year of nursing experience with the population served. Travel may be required; CPR/FA and other certifications are commonly needed.

Qualifications

• Associates degree in nursing or related field required.
• One year of nursing experience with population served preferred.
• Current LPN/LVN licensure in state of residence; CPR/FA and HIV/AIDS certification may be required.

Responsibilities

• Provides basic care and medical treatment under RN or physician direction.
• Completes health assessments and contributes to nursing care plans.
• Monitors and records medications; liaises with pharmacy and administers meds as required.
• Oversees staff administering medication and conducts quality assurance on health records.
• Provides emergency coverage and coordinates with external agencies for services.
• Maintains confidentiality and follows universal precautions; participates in training.
