Job summary

Sevita provides prescribed medical treatment and personal care under RN or physician supervision in home and community settings. The LPN/LVN will perform essential nursing tasks, monitor medications, and coordinate with healthcare teams to support client independence.

Responsibilities include health assessments, medication administration, training staff on safety and infection control, and collaboration with external agencies. Travel may be required; heavy work capacity may be needed.

Qualifications

• Associates degree in nursing or state certificate
• One year experience working with population served in a nursing capacity
• Current LPN/LVN licensure for state of residence
• Certifications such as CPR/FA, HIV/AIDS, BBP and ACT training as required by state licensing regulations

Responsibilities

• Provides basic care and medical treatment to individuals receiving services.
• Completes health assessments and inputs health diagnosis to RN/physician for care plan; follows orders and follows up on appointments.
• Monitors and records medications; acts as liaison to pharmacy; administers medication as required.
• Oversees and monitors staff who administer medication; conducts quality assurance checks on health care records.
• Provides emergency coverage and/or care for individuals receiving services.
• Maintains confidentiality and adheres to universal precautions and Mentor policy.
• Works with external agencies to coordinate services and ensure consistent treatment.
• Administers, tracks, and maintains log for staff and individual's testing.
• Provides training for staff on medication administration, universal precautions and infection control.
• Provides training for individuals on self-medication and prescribed treatments.
• May coordinate household activities; transport individuals to activities and appointments; participates in on-call system.
• Performs other related duties as required.
