Job summary

Sevita is seeking a licensed practical nurse to provide prescribed medical treatment and personal care under RN or physician supervision in a home health setting.

Qualifications

Responsibilities

• Provide basic care and medical treatment under RN/physician direction.
• Complete health assessments and input health diagnoses to care plan.
• Monitor and record medications; liaise with pharmacy; administer meds as required.
• Oversee staff who administer medications and perform quality checks on health records.
• Provide emergency coverage and maintain confidentiality per policy.
• Coordinate with external agencies to ensure consistent treatment.
• Train staff on medication administration, infection control, and safety.
