Job summary

Sevita is seeking a Licensed Practical Nurse in Charlotte, NC to provide prescribed medical treatment and personal care under the direction of an RN or physician.

Responsibilities include health assessments, medication administration, and coordinating services with external agencies to ensure consistent patient care. Candidates should have an associates degree in nursing and active LPN/LVN licensure.

Qualifications

• Associates degree in nursing or state certificate.
• One year nursing experience with population served.
• Current LPN/LVN licensure for state of residence.

Responsibilities

• Provides basic care and medical treatment under RN/physician direction.
• Completes health assessments and inputs health diagnoses for care plans.
• Monitors and records medications; liaises with pharmacy; administers medications as required.
• Oversees staff who administer medication and conducts QA on care records.
• Provides emergency coverage and coordinates with external agencies as needed.
