Job summary

Reema Health in Indianapolis, IN is seeking a Community Health Navigator to build genuine relationships with members and guide them toward greater engagement in their care. You will address social barriers, work with the technology platform daily, and help members meet monthly goals while documenting progress.

The role involves 25–50% travel within a 30-mile community area, Monday–Friday with occasional evenings and weekends. A valid driver’s license and insurance are required.

Qualifications

• 18+ years of age or older.
• Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field.
• Solutions-oriented with strong communication and tech-literacy.
• Willingness to work a remote caseload with occasional in-person meetings.

Responsibilities

• Outreach and enroll members in Reema's care model.
• Build and manage a caseload of 80–120 members and cover for others as needed.
• Assess needs including social determinants of health to create care plans.
• Coordinate referrals and appointment scheduling; document work in Reema App.
• Maintain HIPAA confidentiality and professional boundaries.
• Engage with members through phone, text, email, and in-person visits.
• Connect members to community resources and transport as needed.
