Sea Mar Community Health Centers is seeking a Health Educator I to deliver quality health education services to patients and staff. The role emphasizes preventive care, collaboration with clinic teams, and community engagement to improve access to services.

The Health Educator I will educate diverse populations, maintain materials, and meet productivity goals set by senior management, with bilingual English/Spanish preferred. Vaccination requirements apply for employees.

Qualifications

• BA/BS degree in Health Education, Public Health, Nursing, Education, Nutrition, or related field.
• At least 5 years of health education experience.
• Experience in patient counseling and educating groups.
• Current CPR/First Aid certification or within 90 days of hire.
• TB screening and vaccine requirements including flu and Covid.

Responsabilités

• Assess health education needs and provide education on chronic conditions and preventive care.
• Develop, promote, and evaluate community health sessions.
• Collaborate with local health, educational, and social service providers.
• Attend trainings, complete reports and maintain records.
• Chart notes in Electronic Health Records (EHR).
