Job summary

Neighborhood Health Plan of Rhode Island is seeking a Community Health Coordinator to support the Health@Home home-based primary care program. You will coordinate administrative tasks for a care team, engage patients with complex medical and social needs, and ensure effective communication across nursing, care management, and primary care providers.

Qualifications

• High school diploma or equivalent.
• Two years’ experience in health care setting.
• Proficient in PC and Microsoft Office; EMR experience.
• Ability to communicate with individuals and families from diverse backgrounds.
• Bilingual in Spanish.

Responsibilities

• Answer inbound calls from patients and caregivers during office hours.
• Document in EMR and communicate with community-based care teams.
• Schedule and manage appointments; perform daily reminder calls.
• Register patients and collect updated demographic information.
• Serve as main contact for community-based care teams.
• Maintain accurate records and update EMR within required timeframes.
• Protect patient confidentiality and adhere to HIPAA.
