Philadelphia FIGHT Community Health Centers is hiring a Medical Case Manager to support patients with chronic or complex health conditions and social determinants of health at our Center City clinics. This full-time role focuses on care coordination, benefits enrollment, and trauma-informed support while upholding HIPAA compliance.

Qualifikationen

• Bachelor's degree in a Social Services related field is required.
• 1+ years of related experience.
• Proficiency with HRIS, Microsoft Word and Excel; basic computer skills required.
• Bilingual Spanish/English preferred.
• Strong communication, discretion, HIPAA compliance, and customer service skills.

Aufgaben

• Manage a caseload of clients with chronic or complex health conditions.
• Advocate for clients to access shelter, utilities, food, housing, and programs.
• Assess eligibility for Pennsylvania public benefits (Medicaid, CHIP, SPBP, etc.).
• Maintain regular contact and document care plans in the EMR.
• Provide trauma-informed support and crisis intervention as needed.
• Participate in on-call rotation and cross-coverage as required.
