Responsibilities

• Provide case management services including care coordination, referrals, and resource navigation.
• Conduct patient assessments to identify medical, behavioral, and social support needs.
• Connect patients with housing, transportation, food programs, and behavioral health support.
• Collaborate with clinical and behavioral health teams to develop and implement individualized care plans.
• Maintain accurate documentation in the EHR system and track patient progress.
• Support patient engagement, education, and follow-up to improve health outcomes.
