Job summary

Leoforce is seeking a Case Manager (Community Health) in Albuquerque, NM to provide case management and care coordination for individuals and families served by a federally qualified health center.

The role emphasizes connecting patients with community resources, documenting in electronic health records, and advocating to improve access to housing, transportation, and services. Requires experience with underserved populations and a CPSW preference.

Qualifications

• Experience in case management, care coordination or community service.
• Experience with underserved populations.
• CPSW preferred or similar peer-support credentials.
• Bachelor’s degree preferred.
• Background in community health, behavioral health or social services encouraged.

Responsibilities

• Provide ongoing case management and care coordination for an assigned caseload.
• Assess patient needs and identify medical, behavioral health and social barriers to care.
• Connect patients with community resources and support programs.
• Coordinate referrals for housing, transportation, food assistance, behavioral health and other social services.
• Assist patients with navigating healthcare and community-based systems.
• Develop and maintain individualized service or care plans.
• Follow up on referrals and services to help maintain continuity of care.
• Advocate for patients and address barriers to access.
