Job summary

Aspire Rural Health System is seeking a Community Health Care Worker to support patients in Emergency Departments and inpatient settings by addressing social determinants of health and helping patients access care.

The role collaborates with ED staff, inpatient teams, social workers, care managers, and community partners to connect patients with resources and services. It may require travel between Aspire locations.

Qualifications

• Certification as a CHW/CCHW or ability to obtain within three months.
• Healthcare-related experience in population/public health preferred.
• Experience with Medicaid applications or benefits enrollment desired.

Responsibilities

• Identify and address social determinants of health affecting access to care.
• Collaborate with ED staff, inpatient teams, social workers and care managers.
• Conduct outreach, follow-up calls, and assist Medicaid applications and enrollments.
• Coordinate referrals to community resources and follow up after discharge.
