Aspire Indiana Health is seeking a Community Health Worker - Transportation to support our mission of providing holistic care in central Indiana. This role will coordinate transportation for clients and staff, assist with fleet maintenance, and ensure timely outreach and connections to vital health services.

The CHW - Transportation will pick up clients at their homes, conduct brief outreach calls, and collaborate with our health team to promote a medical home model.

Qualifications

• High School Diploma or equivalent required; Bachelor’s Degree in social work, psychology, counseling or related human service field preferred
• One year of related driving experience required
• Must be able to drive in varying weather conditions
• 15-passenger van or bus driving experience strongly preferred
• Must be able to achieve and maintain Community Health Worker certification within 12 months of hire

Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

Position Summary

Community Health Worker's (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy. The Community Health Worker - Transportation will provide and/or coordinate transportation services to clients and employees and coordinate Aspire fleet maintenance. They will also maintain contact with clients by picking them up at their homes and making telephone calls, promote a medical home model where the CHW, healthcare team and other resources interact to meet the client's needs for services and education and provide referrals as needed.

Education/Experience

• High School Diploma or equivalent required, Bachelor’s Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
• One (1) year of related driving experience required
• Must be able to drive in varying weathering conditions required
• 15 passenger van or bus driving experience and basic motor vehicle knowledge (checking oil, tires for air and tread wear, etc.) strongly preferred

Certification

• Must be able to achieve and maintain Community Health Worker certification within 12 months of hire
