Job summary

Aspire Indiana Health is seeking a Community Health Worker (CHW) – Transportation to support vulnerable populations in Central Indiana. You will provide and coordinate transportation services for clients and staff, coordinate fleet operations, and maintain client contact through pickups and calls.

The role emphasizes coordinating with the healthcare team to meet clients’ needs, offering referrals, and operating in a community-focused, whole-health model.

Qualifications

• High School Diploma or equivalent required; Bachelor’s Degree in social work, psychology, counseling or related field preferred.
• One year of related driving experience required.
• Must be able to drive in varying weather conditions.

Responsibilities

• Provide and/or coordinate transportation services to clients and employees.
• Coordinate Aspire fleet maintenance.
• Maintain contact with clients by picking them up at their homes and making telephone calls.
• Promote a medical home model and connect clients with services and referrals.
