Aspire Rural Health System is seeking a Community Health Care Worker to support patients across Aspire locations. The role focuses on addressing social determinants of health to improve access to care and successful transitions from hospital to home.

Qualifikationen

• Certification as a CHW or willingness to obtain certification within three months.
• Experience in healthcare, hospitals, population health, or community-based services preferred.
• Experience with patient outreach, Medicaid applications, or benefits enrollment preferred.
• Knowledge of social determinants of health and their impact on healthcare access and patient outcomes.
• Ability to work with individuals facing social, financial, housing, insurance, or healthcare access challenges.
• Knowledge of community resources or willingness to learn.
• Ability to work independently and collaborate with interdisciplinary teams.
• Basic computer skills and familiarity with EHR preferred.

Aufgaben

• Support patients in Aspire Rural Health System's ED and inpatient settings by addressing social determinants of health.
• Collaborate with ED staff, care teams, social workers, care managers, and partners to connect patients with resources.
• Conduct patient outreach and follow-up calls; assist with Medicaid applications and benefits enrollment.
• Coordinate referrals to community resources and follow up on identified needs after discharge.
• Travel between Aspire locations as needed to provide support across facilities.
• Help reduce barriers to care and improve care transitions, reducing avoidable ED visits and readmissions.
