Job summary

Aspire Rural Health System is seeking a Community Health Care Worker to help patients access care by addressing social determinants of health, coordinating resources, and assisting with Medicaid applications. The role supports ED and inpatient transitions across Cass City and other Aspire sites.

Successful candidates will have CHW certification or plan to obtain it within three months, healthcare experience, and strong communication skills, with the ability to travel between locations as needed.

Qualifications

• Certification as CHW or CCHW or willingness to obtain within 3 months.
• Healthcare-related experience preferred.
• Knowledge of social determinants of health and their impact on access to care.
• Strong verbal and written communication skills.
• Ability to use electronic health records (EHR).

Responsibilities

• Identify social determinants of health affecting access to care and transitions.
• Coordinate referrals to community resources and assist with Medicaid applications.
• Perform patient outreach and follow-up calls after discharge.
• Travel between Aspire locations to support multiple sites.
