Essentia Health in Brainerd, MN seeks a Community Health Worker to help patients meet personal health goals by addressing social determinants, coordinating with families, and connecting people to community resources.

You will build trusted relationships, enroll eligible patients, provide tailored support for healthcare access, social services, and behavioral health, and advocate for patient needs with the care team.

Formación

• At least 1 year of experience in customer service, social service, healthcare behavioral health or related field.
• Ability to address social determinants of health and work with diverse communities.
• Willingness to obtain CHW certification within 18 months if not already held.

Responsabilidades

• Outreach and enroll eligible patients.
• Build trusting relationships with patients and create personalized wellbeing goals.
• Provide tailored support for accessing healthcare, community connections, and social services.
• Provide referrals for social service resources in the community.
• Advocate for patients' needs and offer social support.
• Coordinate with healthcare teams regarding cultural concerns and barriers.

Conocimientos

Strong written and verbal comms

Educación

Community Health Worker certificate or ability to achieve within 18 months
