Qualifications

• High school diploma or equivalent required; college courses preferred.
• Preferred: ambulatory care experience, community outreach, or knowledge of integrated care.
• Personal experience living with a chronic condition is a plus.
• Motivational Interviewing skills with training within six months of hire.
• Certificate from a Community Health Worker program required within six months.
• Valid driver’s license is required.

Responsibilities

• Bridge between underserved communities and health and social services.
• Provide information and navigation to help patients access care.
• Advocate for patients and community groups with strong public speaking.
• Present to community groups and engage in relationship building.
• Coordinate appointments and assist with plans of care.
