Job summary

Essentia Health's West Duluth Clinic is seeking a Community Health Worker to support patients in the Duluth area. You will outreach, enroll eligible patients, and partner with families to set personalized wellbeing goals while connecting them to healthcare and community resources.

The role emphasizes culturally aware communication, advocacy, and collaboration with the care team to address social determinants of health and improve access to services for diverse populations.

Qualifications

• At least 1 year of experience in customer service, social service, healthcare behavioral health or related field.
• Preferred: strong written and verbal communication skills.
• Knowledge of local community resources and a desire to make a healthy difference.

Responsibilities

Building Location: West Duluth Clinic Department: 1008370 CHNA - EH SS

Community Health Workers (CHWs) are natural helpers and individuals who have a close understanding of the communities they serve. They are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. They creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services. The goal of this team is to improve the patient’s overall wellbeing through developing trusted relationships with the patient and their family.
