Job summary

Essentia Health is seeking a Community Health Worker to support patients by addressing social determinants of health and guiding families toward goal-oriented care. You will reach out to eligible individuals, build trust, and coordinate connections to medical, behavioral health, and community services.

Ideal candidates have at least 1 year in customer service or related fields, strong communication skills, and knowledge of local resources.

Qualifications

• At least 1 year of experience in customer service, social service, healthcare behavioral health or related field.
• Strong written and verbal communication skills.
• Knowledge of local community resources.
• A desire to make a healthy difference in people’s lives.

Responsibilities

• Outreach and enroll eligible patients
• Build trusting relationships with patients and work together to create personalized goals to improve wellbeing
• Provide tailored support in accessing health care, community connection, and social support
• Provide referrals for social service resources in the community
• Advocate for needs of patients and offer social support
• Communicate with healthcare team about cultural concerns and barriers faced by the patients in the community
