Job summary

Lone Star Circle of Care is seeking a Community Health Worker to serve our diverse Texas communities. You will connect patients with resources, address NMDOH needs, and support care coordination across clinics while promoting health equity.

The role requires strong communication, cultural awareness, and a willingness to float between LSCC facilities. A valid Texas driver’s license and relevant CHW certification or timeframe to obtain are preferred.

Qualifications

• High School Diploma or GED required.
• Texas CHW or Promotor certificate required or obtained within 12 months.
• Food Handler’s Certificate required upon hire or within 10 days.
• Bilingual English/Spanish skills preferred.

Responsibilities

• Establish trusting relationships with patients and families.
• Bridge cultural and linguistic barriers to healthcare access.
• Screen patients for NMDOH needs and connect to resources.
• Document patient visits in the EHR.
• Coordinate and maintain food pantry operations.
• Assist patients with LSCC registration forms.
