Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred in health-related field.
• Two years in community health or related field preferred.

Responsibilities

• Coordinate care between patients, providers, and community organizations.

• Identify barriers to care and develop solutions with care teams.
