Qualifications

• High School Diploma or equivalent required.
• Bachelor's degree preferred in Health Sciences, Social Work, Public Health, or related field.
• Two years of experience in community health, patient navigation, or related field preferred.
• Knowledge of social determinants of health and advocacy principles.
• Proficiency with Microsoft Office applications including Excel, Outlook, Teams, and Word.

Responsibilities

• Coordinate care and connect individuals with healthcare providers and community resources.
• Build relationships and empower patients to achieve health goals.
• Educate patients on healthcare resources, preventative care, and support programs.
• Conduct outreach via phone calls, home visits, and community events to engage individuals in care.
• Identify barriers to care and collaborate with teams to develop solutions.
• Advocate for patients with culturally responsive, person-centered care.
• Assist with understanding insurance options and enrollment processes.
• Collaborate with providers to improve care coordination and outcomes.
• Maintain accurate documentation and records.
