Job summary

Centerstone is seeking a compassionate Managed Care Outreach Specialist in Hannibal, MO to connect individuals with healthcare services, navigate insurance and managed care programs, and overcome barriers to care. You will engage patients across outreach channels and coordinate with providers and community resources to improve health outcomes.

Qualifications

• Understanding of managed care principles, healthcare access, social determinants of health, and patient engagement strategies.
• Knowledge of Medicaid, Medicare, QHPs, and enrollment processes preferred.
• Strong relationship-building, customer service, and communication skills.
• Ability to engage individuals from diverse backgrounds and communities.
• Excellent organizational skills with attention to detail.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficiency in Outlook, Excel, Teams, and Word.

Responsibilities

• Conduct outreach to individuals enrolled in or eligible for managed care, Medicaid, Medicare, and other healthcare programs.
• Build trusting relationships with patients through phone, virtual, community, and in-person outreach.
• Educate individuals on available healthcare services, preventative care programs, and wellness resources.
• Promote healthcare engagement by encouraging participation in appointments, screenings, and care plans.
• Assist with outreach campaigns designed to improve health outcomes and care compliance.
• Serve as a liaison between patients, providers, managed care organizations, and community partners.
• Assist individuals in navigating healthcare, behavioral health, and social service systems.
• Identify barriers to care and connect patients with appropriate resources.
• Support care management initiatives, including chronic disease management and patient education.
• Conduct follow-up outreach to ensure individuals receive needed services and support.
