Job summary

CommUnityCare is seeking a Community Health Worker to provide culturally appropriate health education and help community members access the care they need. You will collaborate with the care team, mediate with the community, and empower patients to participate actively in their care management.

Responsibilities include guiding patients to appointments, coaching on self-care, and coordinating referrals to social services while maintaining privacy and complying with procedures.

Qualifications

• High school diploma or equivalent.
• Minimum 1 year of experience in a community health setting.
• Certification as CHW or completion of a 160-hour competency-based CHW training program within the first 6 months of hire.

Responsibilities

• Collaborate with care team to provide patient-centered care.
• Provide cultural mediation between community members and CommUnityCare.
• Empower patients and families to engage actively in their care management.
• Help patients set goals and attend appointments.
• Coach patients in health management and self-care.
• Identify patient needs for referrals to social services to ensure access to care.
