Qualifications

Responsibilities

• Provide ongoing patient advocacy and support to ensure adherence to treatment plans.
• Identify barriers to care and develop plans to address them.
• Coordinate follow-up via phone and in-person visits.
• Explain healthcare information in a culturally sensitive, understandable manner.
• Coordinate care by connecting patients with providers and services.
• Maintain accurate documentation of patient interactions and referrals.
