Job summary

Neighborhood Health Center seeks a Patient Advocate to serve as a liaison among patients, families, clinics, and community services. You will facilitate access to care, provide support and advocacy before and during visits, and help patients understand care plans through outreach and education.

Qualifications

• Requires high school diploma or GED.
• Preferred Associate degree in health-related field (e.g., medical assistant, paramedic, MT, CNA).
• Minimum 6 months in customer service or with the public.
• Spanish bilingual a plus; experience with EHR is desirable.

Responsibilities

• Establish trusting relationships with patients and families.
• Use standardized assessments to determine appropriate interventions.
• Provide advocacy before/during medical visits and assist with care plans.
