Job summary

Oak Street Health is seeking a Community Health Worker to bridge patients, community, and medical systems, promoting health literacy and access to resources. You will collaborate with Medical Social Workers to coordinate care plans and expand resources for patients in Peoria, IL, while maintaining HIPAA standards.

The role requires outreach, strong communication, travel across the community, and dedication to serving diverse populations.

Qualifications

• At least 1 year in healthcare, community-based, case management, or social service.
• Strong oral and written communication skills.
• Ability to manage multiple priorities with a positive attitude.
• Access to reliable transportation and ability to travel throughout the community.
• US work authorization.

Responsibilities

• Establish and maintain strong relationships with patients, community orgs, team members, and partners to coordinate needs.
• Manage patient referrals defined by the care team and collaborate on action plans.
• Facilitate communication among patients, families, caregivers, providers, and community orgs.
• Build relationships with local community organizations to benefit patients.
• Connect patients to state and local resources for housing, transportation, food and daily living needs.
• Assist patients with applications for eligible benefits and resources.
• Promote goal setting and achievement to improve quality of life and self-efficacy.
• Meet patients in centers, homes, or community settings as appropriate.
• Spend about half of time outside the center in patient-centered locations; transport is required.
• Drive engagement with high-risk individuals and accompany to appointments.
