Catholic Charities Brooklyn and Queens has a long-standing commitment to providing social services in Brooklyn and Queens. We are looking for a Community Habilitation staff member to implement personalized plans for individuals with disabilities.

The role requires good communication, flexibility, and problem-solving skills to ensure the health and wellbeing of our consumers. Benefits include generous time off, medical, dental, and retirement savings.

Qualifications

• At least one year of experience serving the disabled population preferred.
• Knowledge of Home and Community Based Waiver.
• Understanding of Person Centered Planning.
• Ability to follow directions and exhibit good decision-making.

Responsibilities

• Implement the Community Habilitation Plan in homes and communities.
• Document individuals’ valued outcomes on billing documents.
• Submit monthly progress notes related to individuals’ outcomes.
• Ensure health, safety, and welfare of consumers at all times.
• Report unusual occurrences to the Supervisor immediately.
• Submit parent/consumer verification forms weekly.
• Submit Bi-Weekly Timesheets to ensure timely payment.
• Communicate any lateness, absences, or schedule changes.
