Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Community Habilitation Skill Developer to train and assist individuals with developmental disabilities in their homes and communities. You will implement individualized plans, document outcomes, and ensure safety while supporting valued goals.

Qualifications

• Must have a High School Diploma or G.E.D.
• AA degree preferred.
• Ability to follow directions and communicate clearly (verbal and written).
• Knowledge of Home and Community Based Waiver is a plus.
• Understanding of Person Centered Planning.
• Flexibility in work hours and responsiveness to scheduling needs.

Responsibilities

• Implement the Community Habilitation Plan in clients' homes or community settings.
• Document outcomes on the billing document contemporaneously.
• Submit monthly progress notes on clients' responses to outcomes.
• Ensure health, safety and welfare of consumers at all times.
• Report unusual occurrences immediately to the Supervisor.
• Submit weekly verification forms to confirm staff presence and service delivery.
• Attend all scheduled meetings and trainings as required.
• Submit Bi-Weekly Timesheets to ensure timely payroll.
• Report to work as scheduled and communicate absences or schedule changes promptly.
