Job summary

Access: Supports for Living in Queens Village, NY seeks a Community Habilitation Worker to provide person-centered supports and promote independence and community integration. The role focuses on guiding daily living skills and respectful engagement within the community.

Qualifications

• High School Diploma or GED required.
• Ability to read, write, speak and understand English.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Provide person-centered supports and services to individuals in home and community.
• Document habilitation services contemporaneously per requirements.
• Support development and implementation of Life Plan and Staff Action Plan.
• Monitor safety and welfare; report concerns to supervisor.
• Assist individuals to build positive community relationships.
• Encourage advocacy for needs and rights; support personal health care.
