Formación

• High School Diploma or GED required; AA degree preferred.
• Ability to follow directions, communicate well, and exercise good judgment.
• Reliability, flexibility, and initiative in scheduling and emergencies.

Responsabilidades

• Implement the Community Habilitation Plan for individuals in home/community settings.
• Document progress and valued outcomes in billing and progress notes.
• Ensure health, safety, and welfare of consumers at all times.
• Report any unusual situations to the supervisor immediately.
• Submit weekly verification forms to confirm service delivery.
