Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Community Habilitation Skill Developer to provide in-home and community-based training for individuals with developmental disabilities. You will implement the habilitation plan, document outcomes, and support families while ensuring safety and compliant documentation.

The role requires flexibility, reliability, and a focus on person-centered planning, with duties spanning scheduling, progress notes, and meetings with the program team.

Qualifications

• High School Diploma or GED required; AA degree preferred.
• Minimum one year experience with the disabled population in home/community settings preferred.
• Knowledge of Home and Community Based Waiver preferred and up-to-date.
• Understanding of Person Centered Planning.
• Flexible scheduling and ability to respond to needs of individuals/program.
• Ability to follow directions, good decision-making and communication skills.
• Ability to build relationships with consumers and families.
• Reliability, dependability and flexibility are essential.

Responsibilities

• Implement the Community Habilitation Plan in homes and communities.
• Document valued outcomes contemporaneously on billing documents.
• Submit monthly progress notes on individuals’ responses to outcomes.
• Ensure health, safety and welfare of consumers at all times.
• Report unusual occurrences to Supervisor immediately.
• Submit weekly parent/consumer verification forms to confirm service delivery.
• Attend scheduled meetings and in-services as required.
• Submit Bi-Weekly Timesheets to ensure timely payroll.
• Report to work as scheduled and communicate lateness/absences promptly.
