Job summary

Catholic Charities Brooklyn and Queens, Inc. is seeking a Community Habilitation Skill Developer to provide training and support to individuals with developmental disabilities in their homes and communities.

Under supervision, you will implement the Individualized Community Habilitation Plan, document progress, and coordinate with families and supervisors to ensure safety and meaningful outcomes. This role requires flexibility, reliability, and strong communication to work with diverse clients

Qualifications

• High School Diploma or G.E.D.
• AA degree preferred
• Experience serving the disabled population preferred
• Knowledge of Home and Community Based Waiver
• Understanding of Person Centered Planning
• Flexible scheduling to meet client needs
• Strong verbal and written communication
• Ability to follow directions and make sound decisions

Responsibilities

• Provide training and instruction to individuals in homes and the community.
• Document contemporaneously the individuals' valued outcomes on the billing document.
• Submit a monthly progress note related to responses to valued outcomes.
• Ensure health, safety and welfare of consumers at all times.
• Report unusual occurrences to the Supervisor immediately.
• Submit weekly parent/consumer verification forms to confirm services delivered.
• Attend scheduled meetings and in-services as required.
• Submit Bi-Weekly Timesheets to ensure staff are paid on time.
• Report lateness, absences or schedule changes promptly to the Supervisor.
