Responsibilities

• Establish and maintain relationships with residents and coordinate community activities.
• Facilitate intake and assessment for new residents.
• Identify barriers to self-sufficiency and coordinate SDOH referrals.
• Develop and maintain a network of education, training, health resources, and partners.
• Document all activities and outcomes in the data management system.
• Advocate for residents and coordinate rent/utility assistance.
• Assist in Adult Education and Youth Education programs.
