Job summary

The Calvert County Government’s Office on Aging is seeking a Community First Choice Supports Planner who can work independently and be able to contribute as a team player.

This position requires establishing and maintaining effective relationships with coworkers, the public, and community partners; communicating effectively in writing and talk; and addressing the needs of the elderly and disabled. A bachelor’s degree and experience are required.

Qualifications

• Bachelor’s degree in social work or a related field.
• Three years of experience with case management and/or working with the elderly.
• Licensed social worker certificate desired; valid driver’s license.

Responsibilities

• Coordinate and track CFC client applications from applicants, families, and providers.
• Provide information and assistance to complete applications and access services.
• Prepare care plans and ensure Medicaid compliance with participating agencies.
• Assign or provide ongoing case management and arrange health, social, and community resources.
• Maintain confidential files and monitor service delivery and budget alignment.
• Recruit providers and outreach to identify potential clients for CFC.
• Report statistics for the program and participate in trainings and meetings.
• Transport clients to appointments and assist with equipment needs.
