Job summary

Youth Advocate Programs, Inc. is seeking a Family Support Specialist to deliver community-based services with 24/7 availability. The role requires a minimum 15–20-hour work week, with 2–5 hours of weekly face-to-face intervention.

You will develop individualized service plans and coordinate with social agencies, hospitals, clinics, schools, and community resources. Responsibilities include counseling, advocacy, and service coordination with families, reporting incidents, and completing

Qualifications

Bachelor's degree in Social Work or related field preferred.
1–2 years of family support or case management experience.
Knowledge of local community resources.
Strong communication and documentation skills.
Reliable transportation and driver's license.
Bilingual Spanish/English a plus.

Responsibilities

Develop individualized service plans with goals from referrals.
Coordinate with social agencies, hospitals, clinics, schools, and community resources.
Provide counseling, support, advocacy, and service coordination with families.
Report incidents and update CYF caseworker on participant activities.
Prepare assessments, closings/aftercare plans, daily progress notes, and billing docs.
