Placements24 is seeking a compassionate Family Support Liaison in Paarl, Western Cape. You will work with families in their homes and communities, helping them access healthcare, education, and social services while advocating for their needs.

The role requires strong communication, empathy, and teamwork, with duties including assessments, developing support plans, coordinating resources, and documenting outcomes to strengthen family well-being.

Qualifications

• Qualification in Social Work, Psychology, Community Development, or related field.
• Strong communication, empathy and active listening skills.
• Ability to work independently and with diverse stakeholders.

Responsibilities

• Conducting initial assessments to understand family needs and challenges.
• Developing and implementing personalized family support plans.
• Connecting families with appropriate community resources, such as healthcare, education, and social services.
• Providing advocacy and emotional support to families throughout their engagement.
• Facilitating communication and resolving conflicts between family members and service providers.
• Monitoring progress and documenting interventions and outcomes.
