Job summary

Tresco, Inc. in Anthony, NM is seeking a Family Service Coordinator to lead enrollment, eligibility services, and support our Home Visiting professionals. You will empower families with resources and coordinate services to help children thrive in the program.

Qualifications

• Bachelor’s degree in social work or related field.
• Valid driver’s license required.
• Background check and driving record check required.
• Bilingual skills are a plus.

Responsibilities

• Coordinate accurate intakes, evaluations, and assessments to determine eligibility.
• Facilitate and actively participate in the development and reviews of Individual Family Service Plans (IFSPs).
• Support families in accessing community resources beyond the child’s condition.
• Coordinate services and monitor delivery, including subcontractors and cross-agency providers.
• Maintain up-to-date and accurate child files in compliance with accreditation and Medicaid standards.
• Facilitate transition planning for children exiting the program.
• Gather and manage a community resource register to better serve families.
• Stay current through required training programs and professional development.
