Job summary

The Arc of Madison County is seeking a Community Support Personnel to implement person-centered plans and help individuals engage in activities within community settings. The role focuses on enabling choices, building social connections, and ensuring dignity and respect in all interactions.

Responsibilities include documenting progress, promoting community participation, coordinating transportation as needed, and maintaining accurate records in compliance with HIPAA and state/federal guidelines.

Qualifications

• Requires ability to communicate professionally, clearly and concisely, both verbally and in writing.
• Ability to adapt to change and prioritize work.

Responsibilities

• Provide services in community settings to engage individuals in activities.
• Assist individuals to participate in community activities and build social connections.
• Assist in identifying opportunities for community participation and volunteering.
• Document records accurately and timely, and maintain HIPAA confidentiality.
• Notify supervisor of any health concerns or safety needs.
