Responsibilities

• Services are provided in community settings.
• Assists individuals in acquiring, retaining, or improving socialization and networking through community resources and participation.
• Helps individuals find and engage in opportunities for community participation, involvement, membership, and connections.
• Encourages meaningful ongoing interactions with community members to develop natural supports.
• Provides service in integrated community settings where opportunities occur and skills are utilized.
• Assists individuals to participate in activities of their choosing.
• Schedules and promotes volunteer opportunities and community connections.
• Observes and documents behavior and maintains timely records and documentation.
• Notifies supervisory staff of health concerns or needs for individuals served.
