Thearcas seeks a dedicated staff member to provide person centered services under the direction of the Person Supported and Fiscal Intermediary. You will follow the Life Plan and Staff Action Plans, ensuring privacy, documentation, and coordinated care for the person supported.

You will advocate for the person, assist with community integration, and maintain timely communications with health teams and the Fiscal Intermediary as part of a quality-focused program.

Formación

• High School diploma, GED, or TABE score preferred.
• Ability to maintain privacy and confidentiality.
• Basic Microsoft Office and computer skills required.

Responsabilidades

• Provide person directed services as outlined in the Life Plan and Staff Action Plans.
• Advocate for the person and support community integration.
• Complete documentation and participate in health-related communications.
