Job summary

SOUTHWEST BEHAVIORAL CARE, INC in Kittanning, PA is seeking a skilled professional to implement and monitor consumer rehabilitation plans and support daily operations between Enhanced Transitional Housing and Enhanced Supportive Housing programs.

Responsibilities include psychosocial assessments, counseling, advocacy for entitlements, maintaining client records, collaborating with the mental health team, and providing on-call support for housing emergencies.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or related human services field from an accredited college or university, or equivalent experience.
• Demonstrated ability to obtain vital information during an interview to make proper recommendations.
• Must obtain criminal history clearances and FBI fingerprint clearances.

• Provide supportive counseling and intensive life skills training to individuals and groups.
• Advocate to access entitlement resources, social services and affordable housing.
• Maintain current client case records and submit required forms and documentation accurately.

• Provide on-call availability for housing emergencies and technical assistance.
