Job summary

The CARE CENTER, INC. is seeking a full-time case manager in Butler, PA to provide case management for behavioral health consumers and coordinate services with formal and informal supports.

The role requires a Bachelor’s degree in social work or related field, ability to obtain clearances, and strong knowledge of mental health programs. You will maintain clinical records, advocates for clients, and ensure productivity as required by state guidelines.

• Ability to obtain vital information during an interview to make proper recommendations.
• Demonstrated knowledge of mental health programs and available social services.

Responsibilities

• Assists consumer with the implementation of CSAS and Service Plan and updates CSAS.
• Maintains accurate and detailed clinical records of assigned caseload.
• Advises supervisory/clinical staff of significant changes in consumer behavior or incidents and responds to emergencies.
• Provides service advocacy to consumers and families and makes referrals to appropriate services.
• Coordinates appointments and links consumers with basic needs such as food and shelter.
• Adheres to policies, laws, and confidentiality requirements.
