Job summary

Citizen Care Inc., a subsidiary of Partners For Quality Inc., seeks a Community Engagement Coordinator in Robinson Township. The role supports individuals with disabilities through interdisciplinary planning, personal outcomes, and rights education.

The position is full-time (40 hours/week) with a flexible schedule to meet program needs, located primarily in daylight hours within Robinson Township. Requires related degrees and experience; driver’s license and clearances are also required.

Qualifications

• Associate’s degree or 60 credit hours from an accredited college or university and four (4) years of work experience working directly with persons with disabilities.
• Bachelor’s degree from an accredited college or university and two (2) years of work experience working directly with persons with disabilities; OR
• Master’s degree or above from an accredited college or university and one (1) year of work experience working directly with persons with disabilities.

Responsibilities

• Identify personal outcomes and provide supports to help individuals achieve goals.
• Demonstrate knowledge of personal outcomes and monitor progress toward them.
• Educate individuals about their rights and help exercise them.
• Coordinate interdisciplinary team recommendations and meetings for service plans.
• Maintain records, write minutes, progress notes and incident reports.
• Assist in staff training to support attainment of personal outcomes.
