Community Connector role supports individuals with health, safety, and community participation, including transportation assistance; requires strong communication and driving license; position is part-time in Silver

Qualifications

• High School Diploma preferred.
• Experience in supporting people to achieve personal goals.
• Must have a valid Driver’s License.
• Ability to lift 60-80 lbs.

Responsibilities

• Communicate status and actively participate in person and caseload.
• Demonstrate respect and acceptance of persons.
• Proactively implement systems to ensure quality services for persons in designated areas such as health, safety, and community participation.
• Provide personal care including eating, bathroom needs, bathing, transfers, grooming.
• Coordinate activities and transportation to support community participation.
