Job Summary

The Community Engagement Coordinator is responsible for providing outreach, education, and recruitment of eligible families for program services in Smith County while maintaining compliance with agency policies and procedures and established state/federal standards and requirements.

Essential Functions
  • 1. Outreach and recruitment of participants for the HEART program.
  • 2. Establish and maintain a trusting and professional working relationship with families by providing regular contact.
  • 3. Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into Salesforce (BCFS) databases.
  • 4. Maintain a list of applications' status and updated the Salesforce database.
  • 5. Provide safe, secure, and supervised transportation for the service population assigned to this position for transport.
  • 6. Provide referrals to participants based on needs identified while conducting outreach efforts.
  • 7. Assist with coordinating and promoting HEART sponsored events and meetings, including classes and community events.
  • 8. Build collaborative relationships with the community, health care providers, community leaders, service agencies, policymakers, funding sources, and target audiences.
  • 9. Facilitate meetings and/or training for program participants.
  • 10. Communicate consistently and effectively with the program director regarding any concerns or issues relating to community outreach efforts.
  • 11. Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook, including demonstrating respect for agency staff, families, and community members and complying with the required dress code, at all times
Measurable Deliverables
  • 1. Documentation entered accurately within two business days.
  • 2. Conduct community outreach a minimum of three days a week.
  • 3. Attend community events at lease 2 times per month.
  • 4. Demonstrate the ability to: a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally in English. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed.
  • 5. Other job duties as assigned per grant requirements.
Requirements
  • 1. Maintain certificates required for the position including but not limited to First Aid and CPR.
  • 2. Valid Texas Driver’s license with a clear driving record and reliable transportation.
  • 3. Pass a pre-employment drug screen and random drug screens.
  • 4. Provide proof or work eligibility status upon request.
  • 5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
