Duane Dean Behavioral Health Center in Illinois seeks an IWC Case Manager to deliver comprehensive case management and education through the Illinois Welcoming Center program. You will engage community members, coordinate referrals, and document all interactions in client files, promoting access to resources and services.

The role requires bilingual Spanish/English, strong organization, and ability to work with diverse populations; position offers full-time salary and benefits in Kankakee, IL.

Qualifications

• Bilingual Spanish/English preferred; bicultural applicants encouraged.
• Experience in case management and outreach essential.
• Ability to maintain accurate records and complete paperwork.

Responsibilities

• Provide comprehensive case management including intake, benefit assessment, goal setting, money management and financial literacy training.
• Engage as a community ambassador; participate in events, schools, meetings and church gatherings.
• Document all contacts and referrals accurately in the participant files.
• Coordinate referrals with staff and ensure follow-up services are provided as needed.
• Link families to community resources and document referral outcomes; weekly case plan development and advocacy.
• Participate in staff meetings and trainings; manage urgent care needs and data reporting.
